JASA provides Palliative Care Support through our Long Island Center for Dignity and Support, an intensive case management program that addresses a broad spectrum of life-sustaining needs on Long Island.
For those facing life-limiting illness, we connect you and your family to a vast network of specialized and culturally sensitive services and resources. With one call, you can reach a professional who can connect you to an array of services, including:
Care management– to assist navigating the health care system and advocating for timely and appropriate care, information and referrals, entitlements counseling, and connections to emergency assistance
Counseling for individuals, couples, families and groups regarding:
- Coping with a serious illness and end of life issues
- Caregiver support
- Goals of care
- Bereavement/loss
- Spiritual care
Community education
Training for professionals
End-of-life resources for clergy and medical professionals
Volunteer opportunities
For those who face economic, physical and social challenges for other reasons, our services promote financial soundness, food security, housing stability, social connectedness, physical/mental well-being, and overall safety through individualized in-home assessment, case management, service coordination, crisis intervention, and counseling/coaching.
