JASANextGen Junior Onboarding

Welcome to JASANextGen Junior!

We're delighted to welcome your child to JASANextGen Junior! This program is designed for third, fourth, and fifth graders to participate in intergenerational volunteer opportunities with the 40,000 older New Yorkers JASA serves each year. 
 

There is a $250 cost to participate in JASANextGen Junior per participant. The full $250 fee represents a charitable contribution.
 

Please fill out this form with your child. The first section will be basic contact information (filled out by you) and the second section will be about your child - please fill it out together.

I hereby give consent to JASA and other entities of JASA to photograph, videotape, and share content from my personal social media accounts on posts related to JASA and older adults. JASA may use, edit, and publish images of me for purposes of publication, promotion, advertising, printing, emailing, copying or web publishing, in any manner or in any medium to perpetuity.

I waive any right to inspect or approve the finished product or to receive compensation arising or related to the use of the photo wherein my likeness appears.

I agree to consider sharing content provided by JASA on my personal social media accounts for the purposes of promotion, advocacy, awareness, and fundraising.
I have read and understand the above PR rights release. I hereby grant permission for my child to participate in JASANextGen. I affirm that I am at least 18 years of age.
